Therapist Cheat Sheet Edition 4
My particular goal in writing this document was to reduce time spent on documentation, while still producing quality and efficient documentation. I have been audited several times in my 12 years of clinical work and have had successful outcomes.
The Therapist Cheat Sheet (version 4 as of March 6, 2023) has been written primarily to assist with language ideas for treatment plans and SOAP notes for clinical documentation for those in private practice. You will find treatment plan GOALS and OBJECTIVE examples to copy and paste for the treatment of Anxiety, Depression, Mood Disorders, Trauma, Adjustment disorder/life transition, Personality Disorders, Grief, Couples, Children/Adolescents, Autism Spectrum Disorder, Substance/Eating Disorders, and ADHD.
You will find general examples of SOAP notes (for each part S, O, A & P) that can be used and edited for your specific clients. Also included are the steps that I took to create my private practice, tips to securing a successful audit, tricks to improve speed of documentaion, common terms to use in treatment plans and notes, suggestions for what to consider adding into your policy paperwork, initial SOAP note, what to include in a discharge summary, what to include in a biopsychosocial, commonly used billing codes and F codes, therapist catch phrases (mainly contributed by members of my facebook group followers), an intake questionnaire example, and some of my favorite resources. I am happy to share my cheat sheet with others and find that writing documentation has become an area that I enjoy hacking and making easier.
